Namaz ke baad ki dua takes two or three minutes and multiplies the reward of your prayer. These are the azkar the Prophet ﷺ said after salam, in the order most people read them.
أَسْتَغْفِرُ اللَّهَ (ثَلاَثَاً) اللَّهُمَّ أَنْتَ السَّلاَمُ، وَمِنْكَ السَّلاَمُ، تَبَارَكْتَ يَا ذَا الْجَلاَلِ وَالْإِكْرَامِ
Astaghfirullah (3 times). Allahumma antas-salam, wa minkas-salam, tabarakta ya dhal-jalali wal-ikram. "O Allah, You are Peace and from You comes peace. Blessed are You, O Owner of majesty and honour."
Subhan Allah 33 times, Alhamdulillah 33 times and Allahu Akbar 33 times, completing a hundred with La ilaha illallahu wahdahu la sharika lah, lahul-mulku wa lahul-hamdu wa huwa 'ala kulli shai'in qadir (or Allahu Akbar a 34th time). The Prophet ﷺ said that whoever does this after every prayer, his sins are forgiven even if they are like the foam of the sea (Sahih Muslim 1352).
Read Ayatul Kursi after every farz prayer. It is the greatest verse of the Quran (Sahih Muslim 1885).
Read Surah Al-Ikhlas, Al-Falaq and An-Nas once after each prayer, and three times after Fajr and Maghrib.
لاَ إِلَهَ إِلاَّ اللَّهُ وَحْدَهُ لاَ شَرِيكَ لَهُ، لَهُ الْمُلْكُ وَلَهُ الْحَمْدُ يُحْيِي وَيُمِيتُ وَهُوَ عَلَى كُلِّ شَيْءٍ قَدِيرٌ
Ten times after Fajr and Maghrib. After Fajr also: Allahumma inni as'aluka 'ilman nafi'an, wa rizqan tayyiban, wa 'amalan mutaqabbala ("O Allah, I ask You for beneficial knowledge, good provision and accepted deeds").

They are read after the farz. In the Hanafi school, if a sunnah follows the farz (as in Zuhr, Maghrib and Isha), keep the azkar short (e.g. Allahumma antas-salam), pray the sunnah, then complete the rest.
Raising the hands in dua is established in general; individual dua after the azkar is fine.
